A Clinical Observation on the Essential Hypertension

Abstract
A clinical observation was done on the 473 patients with essential hypertension who had been treated in the Department of Internal Medicine of Chonnam University Hospital, from 1968 to 1972, and the results are as followings.
1) The most prevalent age group was fifties, and next, fourties and sixties. Male to female ratio was 1.48:1.
2) The mean systolic pressure was 190.7 mmHg, and the mean diastolic 113.8 mmHg. The mean systolic and diastolic pressures were increasing with age, but reverse tendency was observed after sixties in man and seventies in woman in systolic pressure, and after, fifties in both sexes in diastolic pressure.
3) The most frequent subjective, symptom was headache (43%) and the nexts were palpitation, dizziness and dyspnea. All the above symptoms were more frequent in women. Headache was more frequent in lower age group under 49, while dyspnea in higher age group above 50.
4) Retinopathy was noted in 49. % o cases, in which group II (K-W classification) was the most frequent. In relation to diastolic pressure, the higher the diastolic pressure, the more severe tj\he retinal charges.
5) Hypercholesterolemia over 220 mg/110 ml was found in 62.1 % of the cases and showed no relation to age and sex.
6) Proteinuria was noted in 40% f the cases and had a tendency to relate with retinopath y.
7) Complications were developed n 27.9% of all cases. The most frequent complicat on was cerebrovascular disease (15.8%) and coronary heart disease was rare. There was no relatiinship between retinopathy and complications.
8) Abnormal E.C.G. findings were noted in 66.6 of the cases, in which left ventri ular hypertrophy was 48.8%, arrhythmia 7.4%, and the art block 7.4%. In 93.8% of the cases with left ventricular hypertrophy, voltage criteria was satisfied, and STT changes were noted in 45.9% of the cases. Left ventricular hypertrophy was more frequently found in higher diastolic pressure group120 mHg or more)
9) Cardiomegaly in chest roenttenogram was found in 74.2% of the cases and pulmonary congestion in 21.6% of the cases. These findings were also more frequently noted in high diastolic pressure group. (120mmHg or more)
